A 17-year-old died at a Kenilworth centre when he was decapitated by a goods lift.
Superintendent Billy Jones confirmed that the accident occurred late on Friday when someone summoned the lift at Shoprite in the Kenilworth centre.
Terrence Stadler stuck his head into it, but Jones says the lift is not meant for passengers.
The lift, according to the Weekend Argus, suddenly descended, breaking his neck and completely injuring his head.
The accident left staff extremely traumatised, some have received counselling.
The Claremont police are investigating the incident.
