A light motor vehicle collided with a car on Sunday on the R56 in Pietermaritzburg.
After colliding with the cow, the vehicle veered off the road and smashed into a tree.
Netcare 911s Chris Botha said when paramedics arrived, they found that the driver of the vehicle sustained serious injuries and his passenger was in a very serious condition.
Paramedics attempted to resuscitate the passenger but he unfortunately passed away.
Botha said the driver was stabilised on the scene then taken to hospital.
