The Western Cape Government Health Emergency Services responded to an accident in Wellington on Saturday evening.
Two cars were involved in the rear-end collision.
The Western Cape Government health principal communications officer Angelique Jordaan said three ladies were trapped in one of the vehicles and they were freed with the Jaws of Life.
She added that one of the ladies had head, back and neck injuries and was taken to hospital in a critical condition.
The other two females were declared dead on the scene.
Jordaan said the five passengers from the other car, were all stable with less serious injuries.
They were also taken to hospital for further care.
